Disclaimer & Assumption of Risk

Parents of children participating in the Junior Pro Fishermen fishing activity must download, read, and, if in agreement with the contents, sign the Disclaimer & Assumption of Risk form, which states:

I, the undersigned parent/legal guardian of the child(ren) listed above, hereby:

1. Confirm that my child(ren) is/are at least 6 years of age and able to swim.
2. Acknowledge and accept that fishing involves physical activity, exposure to water, hooks, and other equipment, and carries an inherent risk of injury or, in rare cases, death.
3. Agree to supervise my child(ren) at all times during the fishing activity. If I choose to leave my child(ren) at the fishing venue, I do so entirely at my own risk, and I accept full responsibility for leaving them under the care and supervision of Junior Pro Fishermen instructors and their families.
4. Release, discharge, and hold harmless Junior Pro Fishermen, its organisers, instructors, volunteers, and affiliated individuals from all liability, claims, demands, or causes of action that may arise from injury, loss, or death that may occur during the course of this activity, whether caused by negligence or otherwise.
5. Confirm that I have explained the rules and safety measures to my child(ren) and that they understand the importance of following all instructions provided by instructors.
